Sophocles’ Oedipus the King and the Human Condition Oedipus is living in a dream from which he is only just beginning to awake. In this dream, he not only believes that he is in control of his own fate but that he is in control of his own identity. He assumes that he has three virtues: wisdom, reason, and self-control. When he attempts to use these virtues, however, he discovers that he is mistaken on all three counts. His first mistake is believing that he is wise. From this wisdom he hopes to maintain control over the events around him, but true wisdom is actually surrendering to the fact that control is an illusion, a seeming. His second mistake is believing that he is a rational man. Of course, all of these beliefs will come to haunt Oedipus. Oedipus believes that his powers of insight will lead him to the truth and guide him down the correct path, but in fact, he is like a man who studies the ground so intensely right in front of him that he never bothers to look up. The actions that he thought were good will turn out to be bad. He professes his innocence only to discover his guilt. And, in what might be the greatest piece of irony, his cleverness and insight lead him to a contradictory truth—that he, with all his intelligence, is a fool. Oedipus has spent a lifetime looking out, garnishing cognitive power and insight, but now it is time for Oedipus to look inward. We are all blind to the truth until we have the strength to blind ourselves, to start over by seeing the world in whole new way. Oedipus is blind, not only in mind, but also in ears. Philosophers are supposed to be experts on all these subjects. The have well thought out opinions, and they are very learned people. Among the most revered philosophers of all time was Socrates. Living around the 5th century B.C., Socrates was among the first philosophers who wasn't a sophist, meaning that he never felt that he was wise for he was always in the pursuit of knowledge. Unfortunately, Socrates was put to death late in his life. One of his best students, Plato, however, recorded what had occurred on that last day of Socrates' life. On that last day of his life, Socrates made a quite powerful claim. He claimed that philosophy was merely practice for getting used to death and dying. At first, the connection between philosophy and death is not clear. However, as we unravel Socrates' argument backing up his claim, the statement makes a lot of sense. In order for Philosophers to examine their world accurately and learn the truth accurately, they must remove them selves of all distractions. These not only include physical distractions, but they include mental distractions and bodily distractions as well. Philosophers must get used to viewing and examining the world with out any senses. Senses merely hinder and obscure the truth. Sight for example can be fooled easily with optical illusions which occur normally in nature. Sound can be very distracting as well when a philosopher is trying to concentrate. All of these cloud the judgement, and must therefore be detached from the soul. Socrates argues that philosophers must view the world around them with their souls in order to accurately learn about it. However, by detaching their souls from all bodily functions, philosophers may as well be in an induced state of death. In mortem, the soul wanders free and there are no bodily hindrances. Socrates also believed that philosophers look upon death with good cheer and hope. This I find hard to believe because if this were true, the philosopher would not be able to love life, and without the love of life, there is no life to examine and learn about. It is understandable however from another point of view to understand why the philosopher would look forward to death with good cheer. Once the philosopher is dead, his soul is free to roam around without hindrances forever, and all the worlds secrets shall be revealed to him. In fact, Socrates' sees his death as a liberation from the shackles of life for his last wish was for Crito to sacrifice a rooster to Asclepius. This god was normally given sacrifices to free the sickly from the grips of a virus or illness. Perhaps Socrates saw the body as a sickness that fed upon the soul. If this were the case, then indeed Socrates would be happy to leave the bonds of life, for then he could have an eternity to seek out his answers, all without the diversions and distractions of a body. Socrates believed that it was this search that was important, even more so than the answers them selves. Students must be alert to the task at hand, possibly shifting focus to a new activity, and have the mental effort necessary to initiate the task. Students who are able to preview, or think about the outcomes of a task before beginning, are helped in many ways. They can have an idea of what a report will be like once a topic is selected, what materials will be necessary to do an assignment, etc. In addition, students who have a strong sense of step-wisdom – knowing how tasks or activities can be broken down into a series of steps, will be able to determine the first step needed to get started on an assignment. There is an optimal rate for accomplishing most tasks. A competent student is often a well-paced student, performing at a rate appropriate to the task and available time. Taking the appropriate amount of time for a task is largely dependent upon both a student’s temporal-sequential skills and his/her attentional abilities. Temporal-sequential skills help us interpret, retain, or create information that is in serial order. These skills are related to a student’s ability to appreciate time in general and estimate time appropriately. Tempo control (a facet of attention) helps students regulate the allocation of time to the task at hand, and predict the time required for an upcoming task. Tempo control also instills a sense of â€Å"step-wisdom†, the knowledge that it is more effective to undertake activities in a series of steps, rather than all at once. Tempo control allows a student to match his/her pacing to the demands of a given task, e. g. , to take the right amount of time to finish an essay test, to do a homework assignment thoroughly yet efficiently, etc. For many students, time is their most precious resource. Making the most of the time they have enables students to be as efficient and successful as possible, leading a balanced life of work and play. A clear understanding of time is required for students to manage their own time effectively, e. g. , plan long term projects, organize schedules, etc. As such, time management skills are an important component of a student’s success – in school and beyond. In order to meet assignment deadlines and to keep up with schedules related to school and schoolwork, students must engage their temporal-sequential ordering abilities. A strong capacity for work enables students to delay gratification and to persevere through tasks that take considerable energy. It also helps them sustain their effort when information is worthy of attention, even though not immediately exciting, such as when studying a chapter they will be tested on the next day, or reading the instructions for a science experiment. Developing good organizational behaviors can play a key role in efficient school performance. School is much easier for students who know how to organize themselves, their materials, and their work space. Having effective organizational tactics will continue to be a valuable asset throughout a students education and career. Being ready to learn often means being prepared for the next activity, having to shift gears from one task to another, and having all the necessary materials on hand. In order to develop this aspect of learning readiness, students must engage their attentional abilities (especially that of previewing), as well as their memory skills. For example, previewing helps students prepare for a test, get ready for an upcoming class, and smoothly transition from one activity to the ext. Unequal and Unethical Treatment of Women in the Middle East

History shows many instances of discrimination and unequal rights for women. Women have been looked down upon and been abused by men in many different countries and cultures. There is a history for inequity of women, especially domestic violence and abuse. These dilemmas have been going on for the many years in the past, and are still going on today, especially in Middle Eastern countries. One major aspect of the situation of abuse towards women is involved with marriage. In many Middle Eastern countries, it is common for parents to arrange or even force a marriage, and for the children to have no say in it (Child Brides). I believe that this is a large part of the cause for aggression and abuse against the women. In an article from Gaza City called Abuse of Women Still Rife in Palestinian Life, Says Study ¬, a girl named Saana described how she had to endure beating from her father and then her husband as well. Saana’s father arranged a marriage for her, and in the second week of marriage her husband started beating her. He had no apparent reason for beating her. Also, after marrying this man, Saana found out that he had been conducting another relationship with a 15-year-old girl. Her husband eventually dumped her back at her parents’ house. In this culture, divorced women have very bad reputations, no matter what the situation is. Because of this, Saana returned home from her physically abusive husband to verbal abuse from her father. Studies in Gaza show that â€Å"†¦more than one in five women say they suffer physical domestic violence but there is not a single women's shelter. † (Abuse of Women). Although many women are being abused, the government is doing very little, if anything to help them. Even law enforcement officers are very biased. There is basically nothing keeping men from abusing women. Penalties for men who kill or hurt their wives are very insignificant. Also, only male relatives are allowed to file incest charges on behalf of children. Police routinely ‘mediate’ causes of sexual and physical abuse by returning the victims to the ‘care’ of the perpetrators, which obviously doesn’t help the victim at all. Very few women have submitted complaints, though there is so much abuse going on. This may be because they are afraid of their husbands finding out that they complained, and for that reason beating them even more. An article written in 2006 mentioned that of 85 cases of rape in 2003, only in one single case resulted in conviction. (Abuse of Women). According to other online articles, some families arrange marriages for their daughters as young as 7 years old (Child Brides). For example, there was one court case in Saudi Arabia in which a father wanted to allow his 8-year-old daughter to get married to a 47-year-old man. The mother of the daughter was divorced from the father. A relative stated that the mother was determined on getting the marriage overturned. (Eight-year-old girl’s Marriage) There are also some miscellaneous issues about to rights of women, unrelated to marriage. One such example is that women in Saudi Arabia are not allowed to drive cars. A well-known Islamic Scholar named Sheikh Ayed Al-Qarni stated that this rule is based on religious reasons. Al-Qarni set out four clarifying statements: â€Å"One: I do not see women driving cars in our country because of the consequences that would spring from it such as the spread of corruption, women uncovering their hair and faces, mingling between the sexes, men being alone with women and the destruction of the family and society in whole. Two: Sadd Al-Dharaie principle (the closing of doors which could lead to corruption or sinful actions) is one of the values in our religion. Women driving cars is a sinful thing. It is used by those who want to wage a war against purity and hijab. Three: One of the principles of our religion is protecting honor and moral values. Women driving cars would threaten these principl es because of the dire consequences resulting from it. Four: Such public issues must be brought up with the certified religious institution who have the say in such matters as I have said many times before. Though it may be that a religion includes a rule disallowing women to drive, the women should have a right to choose whether to follow this religious rule or not, and not have to endure any legal punishment if they choose not to. Though it seems that the idea of equal rights for women in the Middle East is an impossible notion, there are actually people who care and are working to solve this issue. In Gaza, there have been non-governmental organizations that have been dedicating effort to reduce domestic violence n the area. One such organization featured on BBC News concluded that a major role in domestic violence is the frustration in men after the stress of political and social issues in Gaza. Abu Fahdi, a former abuser, said, â€Å"For us, the war really begins after the military war is over. Here in Gaza men are supposed to be providers. The siege, the strikes, in one way or another they affect all households in Gaza – poverty, hunger, homelessness. Men are really frustrated. They sometimes take it out on their wives. In Gaza, however, there are few places for a woman to run. â€Å"There are no shelters here. Just clinics where women can get comfort, advice or anti-depressants – more often than not, without their husbands knowing. † (Gazan Women). Psychiatrist Suha Mousa works with women that have faced abusive husbands. She says it’s difficult to work with these women because it can get complicated to intervene in their family lives. She explains that if a husband divorces his wife or if she leaves him and returns home to her father's house, she could lose all access to her children. It is thought by many that the Islam religion includes unequal rights for women. This is because the Islam religion requires women to be ‘modest’ and wear headscarves to hide their hair and skin. These restrictions make people believe that women are lower than men. Female member of parliament Jamila al-Shanti disagrees greatly with this idea. She believes that a reason for domestic violence is less-than-adequate amount of respect for women. â€Å"From the moment of birth, a baby boy is celebrated. A baby girl is accepted,† she says. Jamila al-Shanti believes that when people say that â€Å"Islam dictates that women should be at the bottom of the pile,† it is not Islam; it is bad traditions and habits that cause this harsh judgment of women. It is the opinion of human rights groups that laws in Gaza deal with cases of violence much too leniently. They usually consider issues like domestic violence or even cases of husbands murdering their wives family affairs. (Gazan Women) These issues in Gaza, Saudi Arabia, and other Middle Eastern countries cause pain and suffering to be part of every-day life for women. As time passes, people work hard to stop such problems that degrade women. They may never be completely wiped out, but they can be greatly reduced if enough people dedicate themselves towards helping this cause. Women deserve equal rights as much as men do, and this is expressed in the Declaration of Human Rights. Tony Scott, the director, made this film as good as it can get. Following, Top Guns success in box office, with everybody scrambling to watch it, Tony Scott found himself one of Hollywoods best action directors in the 1980s. His best achievement came when Top Gun won an Oscar in 1987 in the Academy Awards in USA. Everybody enjoyed this film, but the question was that did the audience enjoy it? The answer was yes as Top Gun also won the Peoples Choice Award in 1987. Top Gun was released in the USA on the 12th May 1986. Soon later, it had been released in the UK on the 3rd October, 1986. The price-tag stuck on Top Gun had risen over $350 million worldwide and had broken all home-videos record. Top Gun is an action-packed film starring Hollywood star Tom Cruise who is a United States Navy F-14 pilot and plays a fearless character (Maverick/Pete Mitchell). Hes joined by his counterpart Anthony Edwards who plays as (Goose). They are joined by other fighter pilots called Val Kilmer (Ice Man) and Rick Rossovich (Slider) who are Mavericks main rivals. This film it full of action and is about the Cold War between the Americans and the Russians. Throughout the film there is aerobatics performed at the beginning of the film and the end of the film. These performances are shot with great angle-shots and long-shots adding to the effect on the audience. This is what makes the audience raise their necks to see better on their television screen. It also makes the audience feel like they are part of the action with these effects of high-speed planes whizzing across their television screens. The film begins somewhere above the Indian Ocean where Maverick and Goose are flying wingman Cougar and Merlin and are attempting to try and intercept an unknown bogey (enemy). Cougar looks troubled as he gets surrounded by two bogeys. Cougar says to Maverick that Maverick get down here and get these ass-holes off me. Maverick realises this and teases the bogeys recklessly and intimidates them to follow him leaving Cougar with more room. Maverick wasnt satisfied with the amount of fun he was getting so he tilted on his aircrafts head meaning he could see the opponent face to face. He also sticks a finger at him which shows Mavericks attitude. This also shows that in such dangerous and tight situations, Maverick is not just doing his job, but he is also having fun. After losing the bogeys, Maverick heads to search for Cougar, who emerges to be in some discomfort. Stinger (Instructor) orders Maverick to land as he is low on fuel but Maverick looks determined to rescue Cougar. Maverick spots Cougar in deep shock and tries to motivate him by saying Come on Cougar, this is a walk in the park. Cougar also stares at his family photo suggesting that hes afraid of losing his family. He rescues Cougar and flies back to base. Cougar, in such shock, explains to Stinger and says that he has lost the edge to continue anymore. Cougar apologises and hands his pilot badge to Stinger. As Cougar has lost the edge to prolong, this leaves Maverick and Goose to fill in Cougars shoes. But firstly, Maverick is ordered to enter Stingers cabin where Stinger describes Maverick as a brave pilot. But knowing the enthusiasm in Maverick, he is upset with Maverick disobeying a direct instruction to land. But Stinger knows that he has a choice to make after Cougars unlikely resignation. He confirms that Maverick and Goose will be going to Miramar, Fighter Town, USA to fly with the best of the best in the piloting business. Im gonna send you up against the best. You two characters are going to Top Gun. Maverick appears to be over the moon after his success which has sent him to Top Gun. He races an aircraft on his motorbike and clinches his fist while punching the air. This is effective to the audience because when somebody climbs a ladder to reach to a higher status, they usually have to earn it. But in this case, with Cougar retiring, the doors have seemed to just have opened for Maverick to enter without earning it. It also is the turning point in the film. This shows that it was in Mavericks destiny to achieve the Top Gun status. It was an unlikely chance of Top Guns number one pilot losing the edge. After testing his instructors patience on his very first day, Stinger had no other choice other than sending Maverick to Top Gun. When Maverick and Goose enter an off-base bar, Maverick sees Charlotte Blackwood, his new civilian teacher. Unfortunately he is unaware of this and foolishly approaches her with a song. He sings the song called Youve lost that Loving Feeling by the Righteous Brothers. This gives you an idea about Mavericks attitude at work and off work. At work he has motivation, concentration and fun and he does the same at the bar. It feels like he aims a target at Charlotte as if he was aiming a target at an enemy in his aircraft. When he sings to her, Charlotte looks around to see if everyone is looking. And sadly for her, even the off-duty pilots join in. To prevent further embarrassment, Charlotte asks Maverick to sit down with her giving him the eye-contact. As they introduce themselves, they begin a back and forth battle of teasing. As Maverick flies in his plane, he flies to Charlotte only to be given grief. Its clear that Maverick wants to chat up Charlotte but she is not going to make it easy for Maverick. It appears that she teases him by insulting him in many ways. The first example is when Maverick introduces himself she replies Did your mother not like you or something. But then Maverick explains to her that it is his call sign. The other example is when Charlotte asks Maverick if he is a good pilot and Maverick replies I can hold my own. Then Charlotte says Great, then I dont have to worry about you making your living as a singer. These silly remarks add to the audiences effect as Maverick is getting shown up and shows everything is not as easy as it looks. In this film, Charlotte Blackwood seems to change as the film moves on. First she was Charlotte then she turns into a new form called Charlie. This can also symbolise that she has fallen for Maverick and has changed to adapt with him. As the film progresses, a commander introduces Charlie to her students as the new civilian teacher who provide information on the enemy aircrafts and aerial combat techniques. When the commander leaves it to Charlie, she faces the class leaving Gooses eye wide open and Maverick shaking his head. Charlie begins to talk about MIG-28s and states that it cannot do a negative G pushover. Having done an inverted dive with a MIG 28, Charlie looks impressed. So, youre the one. When the lesson finishes, Iceman approaches Maverick asking Who was covering Cougar when you were showboating with the MIG. This suggests that where were you when Cougar was in trouble? Anything could have happened to Cougar in the time when Maverick was having fun. Maverick should have been flying wingman with Cougar but where was he afterwards. Maverick was just having fun by sticking the finger and taking photographs over his enemy. Iceman appears to be jealous of Maverick as he has had more success in his flying till date. Iceman has already criticised Mavericks reckless flying when he was showboating. He again criticises Mavericks flying in the changing room by saying Youre everyones problem. Thats because every time you go up in the air, youre unsafe. I dont like you because youre dangerous. That is what Maverick is all about and he takes this as a compliment towards his greatness in his very short flying career. While all this was happening, Maverick and Goose are instructed to get up to Vipers (boss) office after breaking Top Guns rules of engagement. The first was when Maverick flew under the 10,000 feet limit. The second is when Maverick did that circus stunt fly-by after defeating Jester. Goose is a simple family man, who just wants to fly without any hurdles. You can see the worry Goose is in as he fiddles with his hands by scrunching and rubbing it together. His body language suggests that he is unhappy and scared of what the consequences will be. Viper explains to Maverick that he has broke a major rule of engagement and declares that he should obey them like everyone else does. Lieutenant Mitchell, Top Gun rules of engagement exist for your safety and that of your team. They are not flexible, nor am I. Either obey them or youre history. Vipers status at Top Gun is much larger than Mavericks and Viper shows this by looking outside the window. This suggests that hes not even willing to give eye-contact to his pilots. After Maverick and Goose are dismissed from Vipers office, Viper asks his colleague that if you had to go into a battle, would you want him with you? His colleague responds I dont know, I just dont know. This implies that he knows Maverick has the ability to win but on the other hand, Maverick is risky and put his and others lives at risk. However, if you dont know the answer to a question, you say I dont know once. But he says it twice showing that he really is unsure about the question Viper has given him. In the middle of the night, Goose enters Mavericks room claiming that he cant get to bed. When somebody cant get to sleep, the obvious reason is that they must be in deep thought about something. In Gooses case, it is that he wants to graduate not just blow his career. Goose says When I realised we were off to Top Gun, all I could think of what that trophy. Ive got to be straight with you, Mav. Right now I just hope to graduate. This shows that Goose is not here to mess about and ruin his career; he has a family to think about now. He cant let Maverick do circus stunt fly-bys and breaking rules consistently and get into more deep trouble with Viper. He expresses his feeling to Maverick without hesitation. He says to Maverick that I cant blow this. Maverick has no other family, as with his father is classified as dead, the only person he has close to him is Goose. Maverick knows this and says to Goose Youre the only family Ive got. The next scene is when Maverick and Goose face Iceman and Slider in a volleyball match. Maverick and Goose defeat their main rivals but without realising it, time flies by and Maverick gets late for the date with Charlie. He refuses Gooses will to play one last match and heads straight to Charlies house on his motorbike. The motorbike emphasizes a symbol of rebel. Another example of a rebel is the speed the motorbike is going at. Rebels usually have motorbike because they like the speed they are going at. The camera is shot facing the back of Maverick which includes the Ocean line in the frame. The Blue Ocean line shows you the never-ending desire and shows that opportunity never ends for anyone no matter what circumstances they are going through. When Maverick and Charlie settle down and talk face to face, Maverick talks about his father who had apparently disappeared on November 5th, 1965 in an F-4. This is dramatically effective and leaves the audience sad. Half of the viewers mind thinks that hes dead, but the other half tells you that Maverick will save him. This case study aims to discuss a patient presenting post seizure activity outlining the presenting and foreseen patient problems including a dependant nursing intervention and its role in seizure management. This paper will discuss the presentation of a sixty-one year old Caucasian male, with a complex medical history. His presentation post a witnessed generalised tonic clonic seizure wi ll be explored including the interventions performed by nursing staff. In this case the prescription of Phenytoin was explored as a dependent nursing intervention that aims to provide symptom relief and cessation of seizure activity by binding to inactive sodium channels to prevent neurons firing and therefore prevent muscle contraction (Aschenbrenner Venable, 2009). Thereafter the paper aims to critically appraise the evidence based literature surrounding its use, discussing its effectiveness in the management of the patient discussed and the evaluation of its use. On arrival to the Emergency Department (ED) via ambulance the patient was triaged as a category one and transferred to the resuscitation cubicle for immediate assessment and intervention. The patients history includes alcohol abuse, asthma, epilepsy, heavy smoker and chronic back pain. The patient is generally non-compliant with medications and usually drinks a four litre cask of wine per day of which he has not had for the past three days. The patient had multiple problems which include a Glasgow Coma Scale (GCS) of 8-9, agitation, hypertension, and a threatened airway and therefore required a Nasopharygeal airway. He was noted to continue to desaturate on room air. It can be seen that ensuring the patency of the patients airway is essential in first line management to decrease complications such as tissue hypoxia, hypertension and decreased cerebral blood flow. Thereafter an A,B,C,D assessment approach should be undertaken with interventions aiming to cease seizure activity being a priority (Tesoro Brophy, 2010). A seizure that lasts longer than thirty minutes can have devastating effects on the patient; initially causing an increase in autonomic activity which can lead to hypertension, tachycardia, hyperglycemia, and sweating. However if the seizure continues on greater than thirty minutes, then further deterioration can occur. As metabolic demand during a seizure is high, the body is no longer able to compensate for this. As a result cerebral autoregulation is lost, cerebral blood flow decreases, intracranial pressure rises and hypotension occurs (Tesoro Brophy, 2010). Furthermore if the seizure continues on past this time, death and perman ent brain damage can transpire (Rabinstein, 2010). While the management of a patient having a seizure involves multiple interventions, this case study will discuss one dependant intervention being the administration of IV Phenytoin 1gram in 100mls Normal Saline given over thirty minutes. This decision was made by the treating medical officer as the patient was already taking oral Phenytoin, however was likely that a therapeutic dose range was not achieved due to patient non-compliance. The IV route was chosen as the patient could not swallow tablets and therapeutic levels were required immediately for the safety of the patient (Gallop, 2010). While the patient is already on Phenytoin he is therefore assumed not to be allergic to it, however there can be other side effects to this drug when it is given via the IV route. These include local reactions to the IV site, arrhythmias, dizziness and hyponatremia (Gilad, Izkovitz, Dabby, Rapport, Sadeh, Weller Lampl, 2008). While another study also reports adverse effects of hypotension (Aaro nson, Belgado, Spillane Kunisaki, 2010). For the patient involved these side effects were considered minimal compared to the benefit of stopping the seizure. Numerous studies have been conducted on the use of Phenytoin considering adverse effects, efficacy and in comparison to other antiepileptic drugs. While Phenytoin was used in this case study as the patient was already taking it orally, it was shown to work as the seizure activity ceased. However the study by Zeng, Wang, Xi Yan (2010), illustrate that 39% of patients in their study pulled out due to poor control of seizures. Similar findings were also discovered by Hiba et. al., (2010), who reported less than 40% of patients were seizure free over a twelve month period. Misra, Kalita Patel (2006) also report findings of reduced efficacy of Phenytoin when compared to Sodium Valporate. Ramsay et al., (2010), further substantiates this with more patients leaving the study due to various reasons when compared to that of Topiramate. However, this study then reports that superiority of Topiramate could not be established, with a p value of 0.366. In comparison Gallop (2010), conducted a r eview and found it to be moderately effective for Status Epilepticus (SE) patients and could also be used to prevent early post-traumatic seizures. Turnbull, Howel, Rawlins Chadwick (1985), also report similar findings with efficacy and determined that no statistical difference was found between Phenytoin and Valporate for seizure control, with a p=0.4, both showing similarly good control of seizure activity over a two year period. Several studies have shown a number of adverse effects post the IV administration of Phenytoin in the patient having a seizure. Gallop (2010) and Earnest, Marx Drury (1983), report studies showing burning and pain to the IV site during administration. Tesoro Brophy (2010) also report an adverse effect of pain and oedema to the IV site, however all studies reported pain ceased when the infusion was slowed or diluted further. Other adverse effects were noted by Gilad, et. al., (2008), reporting 12% of patients in the study showed cardiac arrhythmia, vertigo and hyponatremia, however noting a p value 0.035. Further studies show similar findings; Gallop (2010) reports hypotension, bradyarrhythmias and arrest; Aaronson et. al., (2010) shows only hypotension with no statistical difference noted between Phenytoin and Fosphenytoin in terms of adverse reactions. Turnbull et.al., (1985), report on the oral form of Phenytoin and note reactions of rashes, nystagmus, tremor and ataxia. Zeng et. al., (2010), report similar findings with most common reactions being loss of appetite, nystagmus, nausea, fatigue and tremor. Phenytoin was used for the patient to control his seizures as he was already taking its oral form and it was assumed that Phenytoin provided him with good efficacy. However based on the research conducted for this case study, there is no definitive antiepileptic drug that provides excellent efficacy for every patient. A number of studies show several adverse effects relating to Phenytoin and some studies suggest Sodium Valporate as a substitute, which has a limited number of documented adverse reactions. Gilad et. al., (2008) make this suggestion, with 87.8% of patients receiving good seizure control with Sodium Valporate with no recorded side effects, however 88% of patients on Phenytoin also report good efficacy. However other studies, for example Gallop (2010), show good seizure control when administered to patients suffering SE and in post-traumatic seizures. While another study suggests that no superiority was found between Phenytoin and Topiramate in terms of efficacy, however would recommend Topiramate for its reduced number of adverse effects (Ramsay et. al., 2010). While no medication is ever risk free, all adverse effects should be considered and consultation made of whether the risks outweigh the benefits. In this case study the intervention of Phenytoin was decided to be of benefit to the patient, to stop him from having life threatening seizures. While the evidence shows Phenytoin to be of risk to the patient in terms of adverse effects it did stop this patient from seizing while recording no adverse reactions, which suggests good efficacy. The use of Phenytoin, as previously stated, does have its risks, however these can be minimised with recommendations for best practice. Gallop (2010), and Tesoro Brophy (2010), suggest an infusion rate for IV Phenytoin no greater than 50mg/min and monitoring of blood pressure and ECG during and post infusion. Earnest et. al., (1983), make similar recommendations with suggestions of a dilution down to 6.7mg/mL with an infusion rate 40mg/min, with a total dose of 10-15mg/kg, also suggesting regular observations both during and after the infusion is completed. Other studies then suggest a combination of medications. Rabinstein (2010), suggests administration of an antiepileptic drug and also the use of Benzodiazepine. Tesoro Brophy make this same suggestion; stating that Benzodiazepines should be first line and then treat with anticonvulsants. While most studies are able to provide evidence from research carried out, all make the suggestion for further, larger studies, to provide furt her substantiation prior to change of clinical practice. The patient was loaded with IV Phenytoin 1 gram, diluted in 100mls Normal Saline and given over thirty minutes, giving a dilution of 10mg/mL; a slightly higher rate than that suggested by Earnest et. al., (1983), however a slower infusion of 33mg/min, than the suggested 50mg/mL by Gallop (2010), and Tesoro Brophy (2010). The patient was also given Midazolam 3mg in increments for agitation post the seizure, with good effect. The patient at no time became hypotensive or bradycardiac with nil rash, nausea or nystagmus. The patient eventually returned to a GCS 15 and was admitted to the ward for observation. In summary this case study discusses the dependant intervention of Phenytoin administration to a patient that presents to the ED while having a seizure. Various studies report hypotension and bradycardia as life threatening adverse reactions to Phenytoin, however these can be minimised with slow IV infusion. Best practice suggests an infusion rate of 50mg/min with a dilution of 6.7mg/ml. While Phenytoin had good efficacy for this patient, some studies report Sodium Valporate as most efficient, however no antiepileptic drug is found to be superior over all. More research and larger scale studies are recommended prior to application of intervention to clinical practice from the research discussed.

The competition was between the Russian and Canadian figure skating pairs. The Russians showed a performance full of technical difficulty without pulling it off completely. Their performance was marred by simple mistakes. On the other hand, the Canadian pair performed a piece full of emotion, and while not as technically difficult as the Russians, more thorough and precise in their landings and jumps. After their performance the audience and the television commentators all believed they were the gold medallists. However after their score went up, they were sorely put in second place. As it turns out a French judge exchanged votes with a Russian judge so that the Russians would win the event. Since this happened, it h as opened up the doors to the world of figure skating and informed the public of its corruptness. What people need to notice is that judges exchanging votes is only one part of the problem and how well a person actually performs the techniques on the ice is only one part of the judging. In an article published in Newsweek right after the scandal was exposed the author states, "For ages figure skating has attracted ridicule for letting a competitor's nationality, make-up, costume, and choice of music seem to count as much as the athleticism and grace."
